Attendance Verification Form
For online students, these are students taking at least one course online. You must have the Attendance Verification form completed and signed by your professor, with an entry for each course taken off campus, and completed in full for those dates in attendance. The forms are due back to CECP no later than October 15.Forms must be submitted to the CECP Registrar, Shannon Miles or emailed to email@example.com. Emailed forms must be scanned documents. Photographs of the verification form will not be accepted. If the form is not returned to CECP by October 15 the off-campus student will not be able to schedule an appointment with their advisor until the form is returned completed. In addition, the student’s account will be charged the full tuition cost due to no verification of attendance.
