Emergency and Medical Information

Emergency and Medical Information for all students is required for all who attend CEC Parker and College Direct via Highlands Ranch. In order to respond appropriately in an emergency, we need to have a working understanding of your student's health concerns and medical conditions. Please find the appropriate form descriptions below and links to the right for the forms to fill out and turn in before the beginning of the school year. All forms be must returned prior to the first day of school.

Authorization to Administer Medication
This form is to be signed by your healthcare provider giving the Health Office permission to administer any medication in school.  This form must be filled out and signed by a parent/guardian and your healthcare provider if your child needs to take medication during school hours.
 
Colorado Allergy & Anaphylaxis Health Care Plan
When the student has an allergy requiring medication such as Benadryl or an EpiPen, this form is required to be completed by a parent/guardian and your healthcare provider and returned to the CECP Health Office before the first day of school.
Colorado School Asthma Care Plan - When the student has asthma and has a prescription to carry a medication (even if it is not all the time), these forms are required to be completed out by a parent/guardian and your healthcare provider and returned to the CECP Health Office before the first day of school.

Epilepsy/Seizure Action Plan
When the student has Epilepsy and/or Seizure Disorders, this form is required to be completed by a parent/guardian and your healthcare provider and returned to the CECP Health Office before the first day of school.
Diabetes - This is required to be filled out completely, by a parent/guardian and your healthcare provider and returned to the CECP Health Office before the first day of school.

Individualized/Generic Healthcare Plan
When your child suffers from a condition or several other than the ones listed, our health services team need to know how to provide care, what to do in emergency(and know what constitutes an emergency for your child,) and have a good understanding of the student's needs, and medical history. If your child has other health conditions beyond the ones listed above, this is required to be filled out completely, by a parent/guardian and your healthcare provider and returned to the CECP Health Office before the first day of school.
 
Over-the-counter medications
Students are allowed to carry up to 5 days dosage of over-the-counter medications. Medication MUST be in original packaging (bottle/container).  Students may not share ANY medication with other students.
